Hospital Policies

Appointment Policy

Walk-in clients are welcome, however appointments are preferred. Emergency cases shall always receive top priority followed by Patients with previously scheduled appointments.

Vaccination Policy

For the protection of all pets, we require that pets hospitalized, boarded and staying with us short term, to be current on their vaccinations.

Payment Policy

We require full payment at the time that services are rendered. For your convenience, we accept Visa, MasterCard, Discover Card, Care Credit, Cash and personal checks.

We understand that emergencies and accidents are not anticipated.  We are always happy to offer written estimates on all services to be performed and encourage you to be open with your financial concerns and questions.  In the event of unexpected expenses, CareCredit is available.

What is CareCredit? CareCredit is a 3rd-party, flexible patient/client payment program, specifically designed for healthcare expenses, that makes it easier for you to get the treatment or procedures you want and need.  Call us or visit CareCredit online for more information.

Patient Arrival Policy

For your protection, and that of others, all dogs must be on a leash and properly controlled while in the waiting area or exam rooms. 

All cats must be presented in an appropriate cat carrier or on a leash.

Please let us know if you need assistance with your pet.

New Clients: arriving 10-15 minutes early will help expedite paper work and allow our doctor to spend more time with your pet!  It will also help your pet adjust to our staff and clinic.  Filling out a New Client Registration Form (in .pdf format) will help speed up your check in process. Reminders

For your convenience, reminder letters will be mailed to you when your pet is due for vaccinations or routine preventive health care.

Address/Name/Phone

Please help keep our records accurate by updating us with any address, name, or phone number changes. If you notice inaccurate information within your file, please notify us. We depend on current, accurate information to insure you receive your pet's reminders, callbacks, and help us communicate with you better!

Communication/Foreign Languages

Although at this time, none of us at the Animal Hospital of Sun Prairie fluently speaks a second language, we appreciate the fact that in today's world, many of you do.  To aid you in your quest to find out more about us and your pet, we invite you to translate our website into a language you might better understand.
